Life Solutions Club Application & Terms Form

In order to become a member of our club, you MUST fill out this application and the terms & consent form and email, fax or mail these forms back to our office. Without these forms, your membership is not valid. You also must purchase either the premier membership kit ($499.00- includes the premier membership) or the standard member kit ($199.99- include the standard membership). You can purchase either of these memberships online. They are found in the member kit section.
